Qualified Life Events and

Mid-Year Changes

When you experience a major life event, such as getting married or having a child, there are many changes — including updates to your benefits. While you cannot change the plans you are enrolled in, you can change your coverage level as long as you report the qualifying event and make benefit elections within 30 days of the life event. 


NEW!  Required 2-step process for completing your Qualified Life Event:

  1. Obtain necessary documentation based upon the requirements below AND
  2. Schedule an appointment with a Benefits Counselor to complete your benefit changes in Zevo. You MUST speak with a Benefits Counselor to finalize your Qualified Life Event.

Mid-year changes can be made for the following products. See details below:

  • Pet Insurance / Identity Theft: To add, remove, or change coverage, go to Zevo and select "New Life Event".
  • HSA Contribution Changes: Go to Zevo and select "New Life Event".
  • Farmers Home and Auto: Please call 800-438-6381 to speak to a Farmers representative.

Marriage

Action Required Documentation
Add your spouse to your benefits coverage* -A certified copy of your marriage certificate AND -Dependent Insurance Inquiry Form
Add a stepchild to your benefits coverage* -A certified copy of your marriage certificate AND -A certified copy of stepchild's birth certificate

*Please remember to change your beneficiary designation(s) in Zevo if needed.

Birth or Adoption

Action Required Documentation
Add a child as a dependent due to birth -A certified copy of your child's birth certificate (must list name of child & parents OR -Court document (paternity test, divorce decree, court order, etc.) -If the birth occurred within the last 12 months, a hospital certificate is sufficient
Add a child as dependent due to adoption -A certified copy of your child's birth certificate (must list name of child & parents OR -Court document (paternity test, divorce decree, court order, etc.) -If adoption is pending, current Proof of Placement documents are sufficient (statement from adoption agency or court placing the child in the enrollee's home)
Add a child as a dependent due to legal guardianship, legal custody, or responsible for by court order -Current court document assigning minor child to employee under legal guardianship

Divorce or Separation

Gained Other Coverage

Remove benefits coverage for yourself, your spouse, your domestic partner, and/or your child(ren)/stepchild(ren) -Employer letter or other documentation showing gain of coverage date, who gained coverage, and type of coverage gained

Lost Other Coverage

Add benefits coverage for yourself -Employer letter, COBRA letter, or other documentation showing loss of coverage date, who lost coverage, and type of coverage lost

DCFSA - Change in Cost of Childcare

Action Required Documentation
Increase your DCFSA contribution At least one of the following: -Documentation from childcare provider indicating increase in childcare costs -Documentation showing the dependent is newly eligible due to birth/adoption -Documentation showing the dependent is newly eligible due to disability
Decrease or discontinue your DCFSA contribution At least one of the following: -Documentation from childcare provider indicating decrease in childcare costs -Documentation from childcare provider indicating discontinuance of childcare services -Documentation showing that a child turned 13 (no documentation required if the dependent is already enrolled in Medical, Dental, or Vision benefits) -Proof of change in work hours (if new schedule results in childcare being unnecessary) -Documentation showing FMLA start date (if the team member's status is LOA in TCHPoint, no documentation is required)

*Change in DCFSA contributions must be made within 30 days of the dependent's 13th birthday

Death of Dependent

Action Required Documentation
Remove your spouse or domestic partner from your benefits coverage -A certified copy of your spouse's or domestic partner's death certificate*
Remove a child from your benefits coverage -A certified copy of your dependent child's death certificate*

*If reported within 30 days of death, coverage will be terminated on the date of death, and all applicable premiums will be refunded.

 If not reported within 30 days of death, coverage will be terminated on the date of death, but premiums will not be refunded.
