
Qualified Life Events and
Mid-Year Changes
When you experience a major life event, such as getting married or having a child, there are many changes — including updates to your benefits. While you cannot change the plans you are enrolled in, you can change your coverage level as long as you report the qualifying event and make benefit elections within 30 days of the life event.
NEW! Required 2-step process for completing your Qualified Life Event:
- Obtain necessary documentation based upon the requirements below AND
- Schedule an appointment with a Benefits Counselor to complete your benefit changes in Zevo. You MUST speak with a Benefits Counselor to finalize your Qualified Life Event.

Mid-year changes can be made for the following products. See details below:
- Pet Insurance / Identity Theft: To add, remove, or change coverage, go to Zevo and select "New Life Event".
- HSA Contribution Changes: Go to Zevo and select "New Life Event".
- Farmers Home and Auto: Please call 800-438-6381 to speak to a Farmers representative.
Marriage

Birth or Adoption

DCFSA - Change in Cost of Childcare

*If reported within 30 days of death, coverage will be terminated on the date of death, and all applicable premiums will be refunded.
