Benefit Eligibility

Benefit eligibility is determined by the Budgeted Hours (FTE), not by the actual hours the employee worked.

*The Employer Mandate Provision of the Affordable Care Act (ACA): Team members who worked a minimum of 30 hours per week during the measurement period (October 15th – October 14th) are eligible for medical insurance coverage at the full-time rates for the next plan year.   Eligibility for other benefits is not affected. 

 

**403(b) Matching Contributions:  Students, physicians, residents, interns, and temporary employees are not eligible to receive 403(b) matching contributions from TCHHN. All other team members are eligible to receive matching contributions.


Dependent Eligibility

Spouse / Domestic Partners: TCHHN provides insurance coverage for both spouses and domestic partners.  However, if your spouse/domestic partner is eligible for medical benefits through their employer, they are not eligible for medical benefits through TCHHN; you can still enroll them in Dental, Vision, or any of the other benefit offerings with spouse/domestic partner coverage available.


Children: Biological children, stepchildren, adopted children, and any other child you have legal guardianship over are eligible for benefits.  Children of Domestic Partners are not eligible for benefits.


Enhanced Dependent Eligibility for Medicare Eligible Team Members: Medicare eligible team members can waive medical coverage for themselves, enroll in Medicare, and still cover their otherwise eligible dependent on the TCHHN medical plan. Selecting this option may save you money on your medical premiums, while still providing medical coverage for your loved ones.

Dependent Verification: TCHHN requires a one-time dependent verification for newly added dependents. You can go to the

Dependent Verification page to add your dependents after you enroll in benefits.

Medicare Part D Notice* Notice stating whether prescription drug coverage is or is not creditable.
Children’s Health Insurance Program (CHIP)/Medicaid Notice Notice of opportunity for premium assistance for TCH medical plan to eligible employees residing in states with premium assistance programs under Medicaid or CHIP.
Hospital Indemnity Plan Disclosure This notice is applicable to the Hospital Indemnity Plan offered through CHUBB.

Summary of Benefits of Coverage (SBC) and Summary Plan Description (SPD) SBC: Summary documents describing key features of TCH medical and pharmacy plans. SPD: Summary plan document describing benefits, rights, and obligations under the Plan.

Cost Transparency

In compliance with the Transparency in Coverage rule, issued in 2020 by US Departments of Health, Human Services, Labor and Treasury our health plan’s machine-readable files (MRFs) can be accessed via: https://transparency-in-coverage.uhc.com/

 

Once on the UHC website you can access TCHHN specific data buy selecting Ctrl-F on your keyboard, this will give you a search bar, then type in the TCHHN employer name and the associated MRFs will appear.
